An NHS Summary Care Record is an electronic record which contains information about the medicines you take, allergies you suffer from and any bad reactions to medicines you have had. Having this information stored in one place makes it easier for healthcare staff to treat you in an emergency, or when your GP practice is closed.
The NHS wrote to all patients asking if they would like to opt-out, however, if you have recently moved into the area or did not receive a letter, please see the leaflet below.
If you do NOT wish the NHS to hold a Summary Care Record for you (and have not already opted out), please click on the link below and print out the opt-out form and return the completed form to reception. Alternatively, ask for a form at reception.
